PERIORAL LINE FILLER GUIDE
What is perioral line filler?
Perioral line filler places small amounts of hyaluronic acid (HA) filler into or beneath fine lines around the mouth. In some plans, a little filler is also placed along the lip border to give the lines more support from below.
HA is found naturally in the skin and holds water. For fine lines, a soft, flexible filler is usually chosen, because the skin here is thin and moves constantly. Firm fillers placed close to the surface in this area can be visible or feel lumpy.
Perioral filler can soften the look of selected lines. It does not remove them, and it cannot repair sun damage or change the texture of the skin. Very fine, surface-level lines often respond better to treatments that improve skin quality.
Which dermal fillers are used for perioral line filler?
Juvenology Clinic uses Vivacy's Stylage range alongside other dermal filler products where appropriate. Stylage is a range of hyaluronic acid fillers made by Vivacy, with formulations designed for different depths and uses.
Vivacy describes Stylage S as designed for filling facial skin depressions and for lip contouring. The range also includes Stylage Hydro and HydroMax, which are skin-quality products designed to improve hydration and elasticity rather than to fill lines. These are different products with different purposes. Marina chooses the product after assessing how deep the lines are and where they sit.
What causes perioral lines?
The skin around the mouth is thin, and the ring-shaped muscle around the lips (the orbicularis oris) contracts every time you speak, eat, drink or purse your lips. Over many years, several factors can lead to vertical lines:
Natural ageing: the skin produces less collagen and elastin, and becomes thinner
Sun exposure: UV damage is a major cause of skin ageing, including around the mouth
Repeated pursing: drinking through straws, whistling, or pursing the lips while concentrating
Smoking: damages collagen and involves repeated pursing, so it can make lines appear earlier
Lip volume loss: thinner lips give less support to the skin above them
Genetics and hormonal changes, including around menopause
Smoking is only one cause. Anyone can develop perioral lines.
Which perioral lines may filler help?
Suitability depends on the location, depth and character of your lines:
Lines visible at rest that have some depth: may be suitable for filler
Lines linked to thin lips or a soft lip border: filler along the border may help
Very fine, crepey or surface-level lines: often better suited to skin treatments such as microneedling or chemical peels
Lines that only appear when pursing: filler may have little effect
Many people benefit from a combination of approaches over time. Results vary between individuals.
Benefits of perioral line filler
May soften the appearance of selected vertical lip lines

Who may be suitable for perioral line filler?
You may be suitable if you are 18 or over, in good general health, and have vertical lines around the lips that are visible at rest. Good candidates want the lines softened while keeping natural lip movement.
If your lines are very fine and widespread, or your main concern is skin texture, skin treatments are often a better first step. If your lips have thinned, lip filler may be considered alongside perioral treatment. If you smoke, stopping will help your skin more than any treatment can.
Who may not be suitable?
Perioral line filler may not be appropriate if you:
Are under 18
Are pregnant or breastfeeding
Have an active cold sore, infection or inflammation around the mouth
Have reacted to filler before, or had filler complications
Have very thin skin with very fine lines, where filler may be visible
Have certain medical conditions or take certain medicines
Expect the lines to disappear completely

What happens during your consultation
Marina will ask when you first noticed the lines and whether they bother you most at rest, when talking or when wearing lipstick. She will look closely at the depth and spread of the lines and at your lips and skin.
She will review your medical history, medicines, allergies, any history of cold sores, and previous treatments. You will discuss whether filler is the right tool, what it can realistically achieve, the risks, and alternatives within the clinic.
What results can you expect?
Perioral filler may help soften the appearance of selected vertical lines and make the upper lip area look smoother. Deeper lines usually show more change than very fine ones.
The aim is softening, not a smooth, motionless lip. Results depend on the depth of your lines, your skin quality and how much sun damage is present. Marina can talk through realistic expectations.
How long does perioral line filler last?
There is no fixed duration. The JCCP describes typical filler effects as lasting around 6 to 18 months, depending on the product and area. The mouth is one of the most mobile parts of the face, and the soft fillers suited to fine lines may break down sooner than firmer products. Your metabolism, the amount placed and lifestyle factors also play a part.
Recovery and aftercare
Most people go back to normal activities soon after treatment. You may notice:
Swelling around the lips and upper lip
Small bumps along the treated lines while swelling settles
Bruising
Tenderness or redness
Recovery varies. You will receive aftercare advice after treatment. See our aftercare guide for injectable treatments.
Urgent warning signs: contact the clinic straight away on 07413 138 825 if you have severe or worsening pain, or skin on or around the lips that turns pale, white, dusky or blotchy. If your vision changes, call 999 or go to A&E immediately.
Risks and considerations
Risks of perioral line filler include:
Common: swelling, bruising, tenderness and redness
Visible lumps or bumps: filler placed close to the surface in thin skin can sometimes be seen or felt
Bluish discolouration, if filler sits too superficially
Cold sores: treatment around the mouth can trigger an outbreak in people who get them
Asymmetry or uneven softening of the lines
Infection, allergic or inflammatory reactions
Vascular occlusion: the arteries supplying the lips run close to this area. Rarely, filler can block a vessel and damage the skin without urgent treatment
Most hyaluronic acid fillers can be dissolved if needed, after a face-to-face assessment by a prescriber.
