They work through completely different mechanisms and address completely different concerns. Anti-wrinkle injections reduce movement. Dermal filler restores structure. Many patients benefit from both, but they are not interchangeable and treating the wrong concern with the wrong tool produces results that disappoint. The consultation determines which is appropriate for you, and the honest answer is sometimes neither, and sometimes both.

Hyaluronic acid filler is one of the most extensively studied injectable treatments in aesthetic medicine and has a strong safety record when administered correctly. The most serious risk associated with filler is vascular occlusion, where filler enters or compresses a blood vessel and compromises the blood supply to surrounding tissue. In worst cases this can cause tissue necrosis or, in extremely rare cases, visual complications. These risks are real. They are also largely preventable. The product matters. The practitioner matters more.

It depends on the area treated, the product used, and individual factors including metabolism, lifestyle, and how much product was placed. These are averages, not guarantees. Some patients metabolise filler faster. Some areas hold it longer. What I always tell patients is this: results that are maintained consistently over years tend to look better and require less product per session than results that are allowed to fully dissolve before retreatment. Maintaining is easier than rebuilding.

The overfilled look that patients are understandably concerned about is almost always the result of too much product, placed in the wrong anatomical plane, without adequate regard for facial proportion and individual structure. It is a technique problem, not a filler problem. My approach at Juvenology is conservative by default. You can always add more. You cannot take away without dissolving, which is an additional intervention with its own considerations. Natural results come from understanding the architecture of the face, not just filling what appears hollow.

Before treatment, avoid alcohol for twenty-four hours and any blood-thinning supplements like fish oil or vitamin E for a few days, as these increase bruising risk. Afterwards, avoid strenuous exercise, heat including saunas and hot yoga, and direct sun exposure for forty-eight hours. I'll give you a full written aftercare guide at your appointment.

Hyaluronidase is an enzyme that dissolves hyaluronic acid filler. It is the safety net that makes HA filler categorically different from permanent fillers in terms of risk profile. At Juvenology, hyaluronidase is always immediately available during every filler treatment. Always. It is used in two situations. This is why I will only ever use hyaluronic acid filler, not permanent or semi-permanent alternatives. The ability to reverse the treatment at any point is not a minor benefit. It is fundamental to how I practise safely.

Sometimes yes, sometimes no, and the distinction matters. The key question is whether the line is primarily structural or primarily dynamic. Filler placed directly into a line that is driven by movement rather than volume loss will not produce a satisfying result and may look unnatural. This is why the assessment matters as much as the treatment. Understanding what is driving the line determines which tool, or combination of tools, will actually address it.

A wide range, though not every area is suitable for every patient and the assessment determines what is appropriate for your specific anatomy. The tear trough and periorbital area require particular care and are only treated when anatomy specifically supports it. This is one of the most technically demanding areas in facial aesthetics and it is not appropriate for every patient regardless of the concern they present with.

This is a conversation I have regularly and it is one I take seriously, because the consequences of poorly placed filler are real and they affect how patients feel about themselves every day until they are resolved. The first step is always assessment. Before anything else, I need to understand what product was used, how much, where it was placed, and how long ago. Some of this information patients have. Some they don't, which itself tells me something about the standard of care they received. I will always be honest with you about what is achievable and what is not. Not every unhappy filler result can be fully corrected and some require patience while product metabolises naturally. What I can always do is give you an accurate picture of where you stand and a clear plan for what comes next.

There is no single right answer, but there are some clear wrong ones. Dermal filler is designed to restore volume that has been lost and to provide structural support that has declined. In a face that has not yet experienced meaningful volume loss or structural change, filler is rarely the right tool, and placing it before the anatomy calls for it can produce results that look unnatural precisely because they are adding something that was never missing. Starting filler earlier than the anatomy requires it is not prevention. Prevention is sun protection, skin quality work, and regenerative treatments that support the biology before it depletes. The question I ask every patient who comes to me for filler is not how old they are. It is what has actually changed, because that is what determines whether filler is the right answer.
