MARIONETTE LINE FILLER GUIDE
What is marionette line filler?
Marionette line filler is a dermal filler treatment for the lower face. Hyaluronic acid (HA) filler is placed beneath the lines and around the corners of the mouth to replace lost volume and soften the shadow the lines create.
HA is naturally present in the skin and holds water. As a filler, it is a cross-linked gel that stays in place for months before the body breaks it down.
Filler can soften marionette lines and give the mouth corners more support. It cannot remove the lines completely, lift heavy jowls, or tighten loose skin along the jaw.
Which dermal fillers are used for marionette line filler?
Juvenology Clinic uses Vivacy's Stylage range alongside other dermal filler products where appropriate. Stylage is a range of hyaluronic acid fillers made by Vivacy, with formulations designed for different depths and purposes.
Vivacy describes Stylage S, M and L as formulated to fill facial lines and depressions, with different firmness for different depths of line. Some versions contain a local anaesthetic for comfort. Deeper volume loss in the lower face may call for a firmer product. Marina chooses the product after assessing the depth of the lines and the tissue around them.
What concerns can marionette line filler address?
Marionette lines form between the corners of the mouth and the chin. Several changes can contribute:
Volume loss around the mouth and chin, leaving less support beneath the mouth corners
Fat shifting lower in the face, which can contribute to jowls beside the lines
Muscle activity: a muscle below each mouth corner pulls downwards every time you frown or speak
Changes in the jaw bone over time
Skin ageing: less collagen and elasticity, often accelerated by sun exposure and smoking
Filler may help with lines running from the mouth corners towards the chin, shadows beneath the mouth corners, and a downturned look at rest caused by volume loss. Where jowls, loose skin or muscle pull are the main cause, filler may only partly help. Suitability and results vary.
Benefits of marionette line filler
May soften lines from the mouth corners to the chin
Can add support beneath the mouth corners
May reduce a downturned or tired look at rest
Results are visible straight away
Can be combined with treatment of nearby areas where appropriate
Who may be suitable for marionette line filler?
You may be suitable if you are 18 or over, in good general health, and have lines or shadows below the mouth corners that are visible at rest. Good candidates want a softer, more rested look, not a different mouth.
If your main concern is jowls or loose skin along the jaw, filler alone may not be the best option. Our article on jowl treatments explains other approaches. Sometimes treating the cheeks or chin, rather than the lines directly, gives a more natural result.
Who may not be suitable?
Marionette line filler may not be appropriate if you:
Are under 18
Are pregnant or breastfeeding
Have an infection, cold sore, spots or inflammation near the mouth
Have reacted to filler before, or had filler complications
Have significant skin laxity or heavy jowls, where filler may add weight
Have certain medical conditions or take certain medicines
Want the lines removed completely

What happens during your consultation
Marina will ask what you notice about your mouth corners and when. She will assess the lower third of your face in relation to the rest, at rest and in movement, to work out whether volume, skin, jowls or muscle pull is the main factor.
She will go through your medical history, medicines, allergies and previous treatments. You will discuss realistic outcomes, risks and alternatives. If filler is unlikely to help, she will say so.
What results can you expect?
Marionette line filler may help soften the lines below the mouth corners and give the area a smoother, more supported look. Because muscle movement and skin quality also play a part, some softening is more realistic than complete correction.
Results depend on your anatomy, the depth of the lines and what is causing them. The two sides may respond differently. Marina will explain what is realistic for you before treatment.
How long does marionette line filler last?
There is no fixed duration. The JCCP describes typical filler effects as lasting around 6 to 18 months, depending on the product and area. The mouth area moves frequently, which can shorten how long filler lasts. The product used, the amount and your metabolism also make a difference.
Recovery and aftercare
Most people go back to everyday activities soon after treatment. You may notice:
Swelling around the mouth corners and chin
Tenderness when talking or eating
Bruising
Redness or slight unevenness while swelling settles
Recovery varies. You will receive aftercare advice after treatment. See our aftercare guide for injectable treatments.
Urgent warning signs: contact the clinic straight away on 07413 138 825 if you have severe or worsening pain, or skin around the mouth or chin that turns pale, white, dusky or blotchy. If your vision changes, call 999 or go to A&E immediately.
Risks and considerations
Risks of marionette line filler include:
Common: swelling, bruising, tenderness and redness
Lumps or irregularities, sometimes felt when moving the mouth
Asymmetry between the two sides
Overcorrection, which can look heavy around the mouth
Infection, allergic or inflammatory reactions
Vascular occlusion: blood vessels supplying the lips and lower face run near this area. Rarely, filler can block a vessel and damage the skin without urgent treatment
Most hyaluronic acid fillers can be dissolved if needed, after face-to-face assessment by a prescriber.
