What is Melasma?

If you’ve noticed dark patches on your cheeks, forehead, nose, or upper lip that haven’t responded to over-the-counter brightening creams, you may be experiencing melasma.

 

This pigmentation disorder occurs when melanocytes, the cells that produce pigment, become overactive, creating persistent brown or grey-brown patches.

Melasma is often triggered by sun exposure, which reactivates pigment cells, hormonal changes such as pregnancy, oral contraceptives, or HRT, and sometimes a genetic predisposition.

Combination Targeted Approach

The cornerstone of effective melasma treatment is prescription-strength topical therapy. Hydroquinone 4% is considered the gold-standard tyrosinase inhibitor and is usually prescribed for three to six months. Tretinoin, typically 0.05–0.1%, helps increase cellular turnover and improve pigment fading. Azelaic acid, 15–20%, can be used alone or in combination for those with sensitive skin or as an alternative to hydroquinone.

Once your skin has adapted to these treatments, supportive therapies can be introduced. Chemical peels and medical-grade skincare can enhance results and maintain improvements, but they must be carefully selected.
