Professional vs at-home microneedling: why the difference matters

The gap between a professional device and a home dermaroller is not just about price or quality. It is about depth, precision, and anatomical knowledge. And those three things determine whether you get genuine collagen remodelling or slightly better serum absorption.


At-home rollers

These penetrate approximately 0.5 millimetres. At this depth, they temporarily increase epidermal permeability, which improves absorption of whatever you apply afterwards. Useful. But they don't reliably reach the dermis, where fibroblasts live and where collagen synthesis actually occurs. They complement an active skincare routine. They don't replace clinical treatment.


Professional microneedling

Professional devices penetrate 1.5 to 2.5 millimetres, reaching the dermis where genuine regenerative work happens. But here's what most clinics don't tell you: depth shouldn't be uniform across the face.


The forehead skin averages 1.5 to 2 millimetres in thickness. The periorbital skin around the eyes is only 0.5 millimetres. Treat both areas at the same depth and you either under-treat the thicker zones or risk real complications in the thinner ones.


Professional microneedling requires zone-by-zone depth calibration based on actual anatomical understanding of the structures beneath the skin surface.


This is the difference between a treatment that looks like a medical procedure and one that genuinely is one.


What microneedling treats

Acne scarring: the strongest evidence base

Acne scarring is where microneedling's evidence is most compelling, and I want to be specific about this because the data is genuinely impressive. A systematic review of 33 published studies found that every single study demonstrated improvement in acne scar appearance following microneedling treatment. Every one. That kind of consistency across a literature is unusual and it matters.


The mechanism is well understood. Microneedling breaks down the fibrous scar tissue that creates surface irregularity while simultaneously stimulating the new collagen deposition that raises depressed scars. Rolling scars and boxcar scars respond particularly well.


Ice pick scars, narrow and deep, are more challenging and often benefit from combination approaches. Every patient's scarring has a different character, which is why the assessment before treatment matters as much as the treatment itself.


Skin texture, pores, and sun damage

As new collagen integrates into the dermal architecture over weeks following treatment, skin texture improves noticeably. Large pores appear smaller as collagen support around them tightens. Sun-damaged skin with areas of roughness and irregular tone improves as epidermal quality is genuinely restored. Fine lines respond moderately.


Microneedling improves overall skin quality throughout the treated area, which softens fine lines as a natural consequence of better skin rather than targeting them directly.


What microneedling cannot do

I believe in honest expectations, and this matters to me as much as anything else I do in clinic.


Microneedling doesn't produce instant results. Visible improvement takes weeks, with ongoing progress for up to twelve months following a final session. It cannot eliminate deep wrinkles or replace a surgical lift. It is not the right solution for significant skin laxity or severe jowling, where structural support rather than tissue quality is the primary issue. If that's what someone needs, I'll tell them clearly at consultation rather than sell them six sessions that won't address their actual concern.


What it does, genuinely improving collagen density, skin thickness, texture, and resilience over a series of treatments, it does consistently and with an excellent safety profile across all skin types.


Enhancing results: PRP and exosomes

The wound healing cascade that microneedling activates responds directly to growth factors. This is the biological rationale for applying PRP or exosomes immediately after treatment. The microchannels created by needling provide direct access to the dermis, dramatically improving delivery and uptake of these regenerative agents in a way that topical application alone could never achieve.


Microneedling with PRP

Platelet-rich plasma is drawn from your own blood, concentrated, and applied immediately following microneedling. Your platelets release growth factors, PDGF, TGF-β, VEGF, EGF, that directly amplify the collagen synthesis cascade microneedling has already initiated. A systematic review and meta-analysis demonstrated that microneedling with PRP produced statistically significantly greater improvement in acne scar severity scores compared with microneedling alone.


A split-face clinical study found that 43% of patients had an excellent scar response with the combination, compared with 20% with microneedling alone. For scar treatment specifically, the combination is consistently and meaningfully superior.



Microneedling with exosomes

Exosomes deliver a more sophisticated range of cellular signalling molecules than PRP, including proteins, RNA sequences, and growth factors that direct specific repair processes at a molecular level. They represent the most advanced regenerative adjunct available for microneedling, particularly valuable for patients whose goals extend beyond scar improvement to overall skin quality, anti-ageing, and genuine cellular regeneration.

Immediately after treatment, concentrated serums, peptides, PRP, or exosomes are applied depending on your personalised protocol. The microchannels remain temporarily open and this is the optimal window for active ingredient delivery directly into the dermis, bypassing the epidermal barrier that normally limits penetration to a fraction of what we can achieve in this post-treatment window.


Patients leave with noticeable redness, similar to a moderate sunburn. This typically settles significantly by the following day and resolves within 24 to 72 hours. That redness is not damage. It is the visible sign of the repair cascade now underway in your dermis.


Treatment practicalities

Most patients require three to six sessions to achieve their goals, spaced four to six weeks apart. The skin continues improving for up to twelve months after the final session as collagen remodelling completes. The visible results at three months post-final-session are typically better than at one month, which surprises patients who are used to treatments with immediate effects.

Microneedling results are supported and extended by treatments that improve the underlying skin environment rather than simply layering effects on top.

Does microneedling work for acne scars? Yes, with one of the strongest evidence bases in aesthetic medicine. For patients whose primary concern is overall skin quality and ageing rather than discrete scarring, the evidence is less definitive, though the biological rationale for growth factor delivery via microchannels is sound. Can I combine microneedling with other treatments at the same session? PRP and exosomes are applied immediately after microneedling as part of the same session. That's standard protocol and the biology supports it. Combining microneedling with other injectables like Botox or fillers on the same day is generally not recommended. The inflammatory response from microneedling can affect the distribution and behaviour of other injectables in ways that are difficult to predict. A phased approach, treating skin quality with microneedling in one session and structural concerns separately, tends to give better results from both.


Is microneedling safe for all skin types? One of microneedling's significant advantages over ablative treatments like laser resurfacing is its safety across all Fitzpatrick skin types. The systematic review found no cases of post-inflammatory hyperpigmentation associated with standard microneedling, which is a meaningful advantage for patients with darker skin tones who need to be considerably more cautious with ablative procedures. Active acne, open skin infections, and certain medications including isotretinoin and anticoagulants are contraindications that we assess at consultation.
